18G is the thicker of the two nostril gauges and also turns up in earlobes. Threadless is the most secure system to wear in either — which matters most in a nostril, where jewelry goes missing more often than anywhere else.

A flat back is the reason to choose this over a screw or an L-bend. Those styles stay in by catching the inside of the nostril, and that grip fails the moment the jewelry rotates. A flat disc cannot pass back through the piercing at all.
Threadless holds by friction from a slightly bent pin rather than a thread, so nothing works loose overnight. Adjusting the bend is normal — if a top will not stay in, increase the curve slightly at the end of the pin.
Gauge has to match exactly. A thinner 20G piece in an 18G piercing sits loose in the channel and works its way out.
Everything here is implant-grade titanium, ASTM F-136. A nostril heals in 3 to 6 months, a lobe in 1 to 2.
